Do you want to create an answer option that shows both an image and text? This is possible for single-response and multi-response questions:

Create a single-response or multi-response question. Learn more about adding questions.

In the question, go to the "Extra" tab and for a single-response question, check "Radio buttons off" For a multi-response question, check "Checkboxes off"

Under "Answer," add the text for your answer options.

For each answer option, add an image by clicking the image icon next to the relevant option. Select or upload the desired image. Learn more about images as answer options.

Once an image is added, a new field with the text "Replace the answer text" will appear. Click this field and select "Right." This will place the text next to the image.

Save the question by clicking "Save."
